As a Palliative Care Coordinator you will assume 24-hour responsibility for the coordination of care of patients throughout the hospital referred to the Palliative Care Team. The Palliative Care Coordinator will facilitate care of the patient in collaboration with the attending physicians, sub-specialists, nursing and medical teams, and ancillary services. He/She will establish an overall plan and will be responsible for coordinating all supportive care related services, educational and administrative, to ensure comprehensive care to all patients. The Coordinator will also be responsible for ensuring that the palliative care services for Methodist Healthcare System are in constant compliance with JCAHO.
