Position Summary:
 

Performs behavioral health triage and review of inbound calls from individuals requesting medical advice to ensure that services rendered to customers meet clinically appropriate direction and triage. Utilizes clinical skills to coordinate, document, and communicate all aspects of the appropriate clinical advice and triage based on criteria and in adherence to adopted protocols. Clinical triage includes all levels of acuity of medical presentations. Assists callers with coordination of care and appropriate urgent and emergency care. Validates and interprets medical information from callers using evidence-based criteria sets. Consults with SYHC medical providers on non-emergent cases where protocols are not clear. The Behavioral Health Triage RN will directly supports SYHealth highest risk patients. In collaboration with other members of the healthcare team, the Behavioral Health Triage RN is responsible for organizing, coordinating, and providing care coordination and care management services to patients within the practice who are most at risk for health deterioration, sentinel events, and/or poor outcomes.
 
Essential Functions of the Job: 
 
1. Utilizes the Nursing process and clinical protocols to assess, analyze, plan, implement and evaluate patient’s clinical concerns. 
2. Assesses patient based on the patient’s own evaluation and responses to questions.
3. Demonstrates outstanding telephone communication and customer service skills. Speaks with a pleasant, professional voice. Portrays a positive, caring and helpful tone and manner with each call. Communicates clearly and concisely both verbally and in writing.
4. Performs administrative assignments such as filing, answering phones, typing, and completing progress notes in the computer system.
5. Utilizes a critical thinking approach to gauge risks, determine acuity of caller’s situation, problem solve and make decisions. Applies analytical reasoning, reflection and rational problem-solving skills, using verifiable information, and clinical judgment, in order to choose among alternative solutions to clinical problems.
6. Provides timely, efficient, and accurate triage responses and documents all calls and information discussed. Records and refers all pertinent information accordingly. Documentation in NextGen is timely and concise.
7. Administers and notes reactions to psychotropic drugs and other medications.
8. Arranges further medical attention when necessary.
9. Assists the client in understanding the nature of emotional disturbances and accepting the need for treatment.
10. May collaborate with other resources such as behavioral health clinicians or crisis staff to best direct callers.
11. Provides clear patient information and education to the patients and documents patient understanding.
12. Provides referrals to other programs or resources when appropriate.
13. Advises manager of trends in calls and potential quality of care and legal issues.
14. Meets departmental and/or regulatory turnaround times for clinical triage while maintaining productivity and quality standards.
 
Additional Duties and Responsibilities
 

1. Maintains established departmental policies and procedures, objectives, quality assurance program, safety, environmental and infection control standards.
2. Enhances professional growth and development through participation in educational programs current literature, in-service meetings and workshops.
3. Attends meetings as required.
4. Performs other related duties as assigned or requested.


Requirements

Job Requirements:
 
Education Required (Minimum level of education): Graduate of an accredited school of
Nursing.
Certifications/Licenses Required: Current CA RN and driver's license. CPR certification.
Experience (Minimum level of experience): Minimum of 2 years Behavioral Health RN clinical experience.
Verbal and Written Skills Required to Perform the Job: Bilingual (Spanish-English). Outstanding communication and telephone skills.
Technical Knowledge and Skills Required to Perform the Job: Basic Microsoft computer skills/knowledge.
Equipment Used (Computers, phones, vehicles, copy machines and fax machines etc.): Office equipment such as computers, copiers, telephone, fax machines, etc.
Working Conditions and Physical Requirements: Frequent prolonged sitting.
Handles emergency or crisis calls. Occasionally subject to multiple calls and inquiries.
Working Conditions: May work variable schedule and/or extended hours.
