Penn Medicine at Home is now offering 2 tracks for RN employment: This position may be filled at the Clinical Nurse - NCL or Clinical Nurse II track, depending on degree level and years of experience.Penn Medicine At Home provides patients with necessary treatment and care in the comfort of their own home, which can lead to more flexibility and independence in their daily routines. Our multidisciplinary team approach allows us to provide continuity of care from hospital to home.Our home care services are rapidly expanding, making this a perfect time to join a team focused on building relationships with patients and utilizing strong clinical skills to improve patient outcomes in the home.These positions offer the following:¢ Autonomy while working with supportive and caring managers for your training and development¢ Intimate treatment setting: All care is provided in a 1:1 ratio.¢ You will function as a true partner in patient care: Educating patients and caregivers in proper care techniques is our primary goal.¢ Ability to utilize clinical skills obtained from the acute care or inpatient settings to keep your skills sharp.¢ Work life/balance: schedule Monday- Friday day shift; 1 weekend per month; 2 holidays/year;The Penn Care at Home Clinical Nurse provides care for patients in their homes, at every stage of the lifespan, in Philadelphia and the surrounding counties.  Incumbent is a premier provider of quality, compassionate and innovative nursing services in the home. This care allows patients to recover at home from a variety of diagnoses; including congestive heart failure, orthopedic surgery, diabetes, stroke, cancer, and respiratory illness. Additionally, incumbent is responsible for practicing within the home care regulations and reimbursement criteria.The Clinical Nurse 2 or Clinical Nurse-NCL is an RN that practices as a full partner on the health care team and consistently demonstrates safe practice within the organizational professional practice model. The Clinical Nurse 2 or Clinical Nurse-NCL possesses the knowledge, skills and the attitude to effectively meet standards for competent professional nursing practice as specified in the Clinical Nurse 2 position description addendum.The Clinical Nurse 2 or Clinical Nurse-NCL exhibits UPHS organizational behavioral competencies which reflect its mission and are embedded in the professional practice competency domains. The Clinical Nurse 2 or Clinical Nurse-NCL independently provides patient care through the application of the nursing process and accepts accountability for the nursing care of assigned patients. Assigned patients may range in age from infancy to the elderly.The Clinical Nurse 2 or Clinical Nurse-NCL demonstrates within the competency domains of Continuous Quality Improvement, Evidence Based Practice and Research, Leadership, Person and Family Centered Care, Professionalism, Safety, Technology/Informatics, and Teamwork (Definitions listed below). The Clinical Nurse 2 or Clinical Nurse-NCL works closely with the interprofessional health care team to facilitate the coordination of care across the continuum. The nurse focuses on expanding the knowledge and skills necessary to provide individualized care based on physical, psychosocial, cultural, educational, safety, and age appropriate considerations of assigned patients. The Clinical Nurse 2 or Clinical Nurse-NCL seeks feedback in assuming leadership roles and provides feedback for improved clinical practice. The nurse demonstrates how nursing practice impacts the organizational vision, mission, and goals and the care delivery system. The nurse complies with all regulations and standards of regulatory and accrediting bodies.

BSN from a NLN approved school of professional nursing and current PA RN licensure required.  Candidate must possess at least one year of clinical hospital based or home care experience. Incumbent must have a current, valid driver's license and car insurance, and be able to drive long distances.  \n\nThe PCAH Clinical Nurse must be able to function effectively under stressful situations and tight deadlines; must be self-directed and a self-starter; must demonstrate creative thinking ability; must demonstrates the knowledge and skills necessary to provide the expected standards of care based on physical, psychosocial, cultural, educational, safety, and age appropriate considerations of assigned patients; must demonstrate the professional skill and judgment necessary to provide care within the more autonomous setting of the patient’s home or at the various sites of hospice care; must attend to the values and rituals important to the patient-family-community environments as well as upholding nursing practice and safety standards.
