Penn Home Infusion Therapy is seeking a Nursing Care Plan Supervisor to join our team in New Jersey. This is a field based, per diem day shift opportunity. The ideal candidate will be available on a per diem basis to work a flex schedule in accordance to need. NJ Territories/ Counties: Burlington, Cumberland, Salem, Gloucester, Camden. (territories/counties assigned in accordance with need and proximity).As a Nursing Care Plan Supervisor with Penn Home Infusion Therapy, you will be accountable for the planning, coordinating, and delivery of quality care and services, maintaining supervision and direction of nursing and paraprofessional services. The supervisor takes reasonable steps to ensure that services are available; care and services provided by the staff are coordinated and integrated; policies and procedures that guide and support the provision of care and services are developed and implemented. Responsibilities are performed in compliance with Agency policies and procedures and applicable duties including 30 and 60 day personal and infusion care plan supervision as appropriate to the patient population, as well as Federal, State, and local regulations.Program Overview:Penn Home Infusion Therapy offers comprehensive, wrap around care, managing all aspects of a patients infusion therapy in the home. This includes:¢ Best-Practice, disease specific care teams that allow for expertise across oncology, nutrition, infectious disease, cardiology, and pain management¢ Customized patient care plans developed by a multidisciplinary team that includes nurses and infusion pharmacists¢ Collaboration with Penn Medicine at Homes comprehensive programs to meet individualized patient needs¢ Easy-to-read visual training aides that are therapy specific for patients and their families¢ Nurses and managers available 24 hours a day / 7 days a weekPenn Home Infusion Therapy's home care and home infusion nurses are specially trained and certified to administer all infusion therapies in the home. Our nurses are all able to maintain PICC (Peripherally Inserted Center Catheter) lines and to work with central venous access devices. Our pharmacists and pharmacy staff maintain vital communications with a patient's physicians, ensuring timely, precise care.¢ Infusion therapy available through Penn Medicine at Home includes: IV antibiotics, Biologics, Chemotherapy, Nutrition, Hemophilic Therapies (Factor), Immune Globulin, Milrinone and Steroids.¢ Supportive Therapies: Anti-emetics, Colony Stimulating Factor, Hydration (IVF), and Pain Management (PCA).Some job functions and duties will include, but are not limited to:¢ Provides over-site for clinical staff on the following: clinical staff case management & direct clinical care, regulatory compliance, review of new products and services, and coordination of orientation activities between team and nurse educations.¢ Assures that the staff is performing their duties and responsibilities according to their position description. Completes all probationary and annual performance reviews for all field staff. Ensures that staffing and scheduling provide safe, effective, and high-quality care. Ensures that all regulatory requirements are met and that staff is fully utilizing materials, staff, and financial resources.¢ Performs and documents the initial evaluation visit, reassessment, and supervisory visits for all patients. Maintains current and accurate patient records.¢ Builds strategic, clinical, and business literacy through staff education and communication. Builds measurement systems to track performance improvement initiatives. Fosters the continual integration of UPHS research and education activities into clinical operational processes.¢ Conducts orientation program for new PMAH employees. Demonstratesregard for the dignity and respect of all patients, families, representatives as well as employees, volunteers, and medical staff.¢ Performs chart reviews to ensure compliance with State Department of Health and JC regulations as appropriate.¢ Provides patient care/case management to a small group of patients as a method of ensuring clinical competency.¢ Performs duties in accordance with Penn Medicine and entity values, policies, and procedures¢ Other duties as assigned to support the unit, department, entity, and health system organization

Educational Requirements:\n• Bachelor's Degree in Nursing – required\n\nLicensure, Certifications, and Registration Requirements:\n• Active Registered Nurse currently licensed by the State of New Jersey – required\n• CRNI Certification - preferred\n• OCN Certification - preferred\n\nWork-Related Experience:\n• 2+ years of progressive nursing experience in the setting of home care, home health agency/home infusion – required\n• Leadership experience in home care, home health agency/home infusion setting - required
