Penn Medicine Princeton Health is seeking a Full-Time Heart Failure Coordinator to join our Nursing Administration Department in Plainsboro, NJ.The Heart Failure Coordinator works with the interdisciplinary health care team to facilitate patient understanding of the disease process and to promote care based on evidence based clinical practice guidelines. The position requires expert communication skills and the ability to work collaboratively with a multi-disciplinary health team. The Heart Failure Coordinator maintains a strong component of patient advocacy and provides both formal and informal education for professional, technical and ancillary personnel. Some job functions and duties will include, but are not limited to:¢ Evaluates identified patient records for compliance with evidence based standards of care by reviewing physician and nursing documentation and diagnostic studies¢ Initiates face to face communication with nurses, attending/consulting physicians and other health care providers to discuss, confirm or make suggestions regarding patient management and evidence based standards¢ Initiates or recommends referrals (consultations) to physicians and/or other health care providers (social services, case management, pharmacy, palliative care, nutrition, physical therapy, cardiac rehabilitation, etc.) and provides information regarding community health programs to support risk management (smoking cessation classes, etc.)¢ Interfaces with family, therapists, case managers and social workers as needed, to identify specific discharge needs, facilitating optimal care transition and preventing re-admissions, Emergency Department visits and unscheduled office visits¢ Takes accountability of communication for post discharge follow up phone calls to patients discharged with a primary diagnosis of HF. ¢ Identifies opportunities for and provides both formal and informal education for nursing and other health care providers

Educational Requirements:\n• Graduate of an accredited school of nursing; Bachelor's degree in Nursing (BSN) - Required, or in pursuit of.\n• Masters Degree - Preferred \n\nRelated Work Experience:\n • 5-8 years relevant experience - Required \n\nLicensure, Certification or Registration Requirements: \n• RN licensure in the State of NJ - Required \n• Must document a minimum of 8 hours of Heart Failure specific education, and an additional 8 hours of other cardiovascular related education annually.- Required\n \nSpecialized Skills & Knowledge Requirements:\n• (CHFN) Heart Failure Certification through The American Association of Heart Failure Nurses - Preferred \n• Familiar with Epic documentation - Required \n• Familiarity with (GWTG) Get With The Guidelines - Stroke American Heart Association - Preferred \n• Excellent interpersonal and communication skills\nEffective teaching skills
